25 Years of Action: Reflecting on To Err is Human and the Leapfrog Legacy

Join us for a powerful panel marking 25 years of progress since the founding of The Leapfrog Group, born out of the landmark To Err is Human report. Published in 1999, the report sparked a national movement for patient safety and accountability. This conversation brings together leaders in health care quality, policy and patient advocacy to reflect on the strides made, examine persistent challenges and reignite the call to action for a safer, more transparent health care system.

AI and Patient Safety: Promise, Pitfalls and the Path Forward

Artificial intelligence is rapidly transforming health care—but what does it mean for patient safety? This panel will explore how AI is being used to reduce errors, improve outcomes, and support clinical decision-making, while also addressing the risks that come with emerging technologies. This session will provide an in-depth review of the differences between the Survey and Safety Grades and the measures from the Survey that are used to calculate the Safety Grade, including CPOE, BCMA, ICU Physician Staffing, Nursing Workforce, the NQF Safe Practices, and NHSN infection data.
