Join us for a powerful panel marking 25 years of progress since the founding of The Leapfrog Group, born out of the landmark To Err is Human report. Published in 1999, the report sparked a national movement for patient safety and accountability. This conversation brings together leaders in health care quality, policy and patient advocacy to reflect on the strides made, examine persistent challenges and reignite the call to action for a safer, more transparent health care system.

Deep Dive into the Leapfrog Hospital Safety Grade

Have questions about the differences between the Leapfrog Hospital Survey and Hospital Safety Grade? Need help figuring out how Safety Grade scores are assigned to Leapfrog Hospital Survey Results?
This session will provide an in-depth review of the differences between the Survey and Safety Grades and the measures from the Survey that are used to calculate the Safety Grade, including CPOE, BCMA, ICU Physician Staffing, Nursing Workforce, the NQF Safe Practices, and NHSN infection data.
This session will also include a review of the Safety Grade methodology and key dates, and overview of the Hospital Safety Grade calculator.
